Join our team as a Community Paramedicine Program Provider, where you will play a crucial role in improving healthcare access and outcomes for underserved populations. This role allows certified Paramedics, EMTs, and LPNs to operate in an expanded capacity, delivering public health, primary, and preventive care services to patients and communities. Success in this role involves strong communication, independent work, and the ability to address patients’ needs through one-on-one care in their homes. You will be part of a growing initiative that bridges healthcare gaps and empowers individuals through education and community resource referrals.

  • Conduct in-home visits for recently discharged patients to assess health needs and identify barriers to care.
  • Perform blood draws, wound care, and other essential health services during home visits.
  • Provide patient education, health empowerment, and referrals to appropriate community resources.
  • Collaborate with healthcare providers to enhance care coordination and reduce duplication of services.
  • Utilize effective time management to complete all tasks independently and efficiently.
  • PA-certified Paramedic, EMT, LPN, or PHRN.
  • High school diploma or equivalent.
  • Valid driver’s license.
  • CPR, ACLS, and PALS certifications.
  • IBSC CP-C certification (required within 1 year of completing CP training).
  • Experience or training in Community Health, Public Health, Social Sciences, or Case Management (preferred)
  • Competitive pay: $21.50 – $27.50 per hour.
  • Flexible schedule options (8-hour or 10-hour shifts, Monday to Friday; 4 or 5 day work week).
